It is very important that you, the patient, understand that an appointment time is set aside especially for you. We make every effort to honor all time commitments and request that you extend the same courtesy to us.
If you cannot keep an appointment, please notify us immediately. We ask that you please give us at least a 24-hour notice. This courtesy makes it possible to give your appointment time to another patient who desires to see the doctor.
We recognize that you can have an emergency situation arise and we will do our best to respond to your problem promptly. If such a problem arises, please call us as early in the day as possible. If such a problem arises when the office is closed, and you are unable to reach the doctor, contact the nearest emergency care unit.
Regular Visits / Recall
We hope you share in our belief that regular preventive dental care is a sound investment. The mouth is our means of eating, speaking and showing emotion. It is also a major factor in our general appearance. While the responsibility for returning for treatment rests primarily with you, we provide the service of a phone call or written reminder when it is time to return. At your appointment, please advise us of any changes, such as a new address, phone number, insurance carrier or health matter.
Fees and Payment Policy
In our effort to keep dental costs down while maintaining a high level of professional care, we have established a variety of payment options. Fees may be paid through any of the following means:
Cash • Check • Charge by Visa, MasterCard, or Discover • Care Credit (Pre-Approved Payment Plan)
Accounts outstanding more than 60 days from the initial billing date will bear 1½ % interest per month (18% APR).
On your first visit with us, we ask that you bring in your insurance card. We provide thereafter the courtesy of filing the insurance claim for you. Your insurance contract is between you and your insurance company. Please remember that no insurance company attempts to cover all dental costs. Some companies pay fixed allowances and others pay at a percentage. It is your responsibility to pay any deductible and other balances not covered by your insurance company.
Your estimated portion is due at each dental visit. If for any reason your insurance company has not paid, you are responsible to pay your balance within 60 days of the initial billing.
To our non-insured patients we offer a membership plan with a yearly fee to reduce fees for you and your family.
Everyone in this practice operates as a team member. As trained professionals, we take pride in providing quality dental care through continuing education, expanded functions and sterilization.
